Bleeding Disorders Association Clinic Travel Assistance Program Please make sure when you attend your yearly clinic appointment that you ask for the CTAP postcard to be signed by an HTC staff member. Using the preaddressed envelope, mail the postcard back to the Bleeding Disorders Association of Northeastern New York, (BDANENY) and you will receive a check in the mail for round–trip-mileage from your home to the clinic appointment. This offer is good once per year until further notice. (Limitations apply) Purpose: The Bleeding Disorders Association of NENY, Inc. (BDA) is initiating this program to financially assist families in transportation cost to and from their scheduled Hemophilia Treatment Center appointment. Program: The above organization will offer to pay the mileage cost of the family and/or patient attending their regularly scheduled Hemophilia Treatment Center (HTC) visit at the Albany Medical Center. The cost of mileage will be based on the General Services Administration's privately owned vehicle automobile reimbursement rate for that year. Reimbursement will be for round trip mileage from the family's legal home address to the HTC and back home. This round trip distance will be limited to 100 miles per family and will be offered for one clinic visit per calendar year. Confirmation of the attendance and completion of the regularly scheduled HTC clinic visit will be sent in writing to the BDANENY by a member of the HTC staff. Mileage will be estimated in a method to be determined and a check for the cost of travel mileage will be sent to the individual's legal address. This program may be terminated at any time at the discretion of the BDA. | ||||
